@geoffrey-gordon Gravity forms can do all that. It’s just drop down boxes.

@geoffrey-gordon this can be done most probably with a custom plugin. You will have to add all the info in the database and then use a similar form to search for it.

I’ve done something similar for a client using Gravity Forms.

I have no idea what plug-in might accomplish your task as no plug in will verify your details.  This would  be programmed as an “if this, then this” type item.  So even if you find a plug in the variables will be up to you to fill in.  I’ve never heard of a plug-in like what you are asking for.

Which certainly does not mean there is not one.
