I want to find someone (or a program) who can build an interactive form that would sit in a secure area on our website. It will be a large interactive document that takes customers through a series of questions and answers. As they fill in the answers into the various fields these answers need to automatically copy/populate other fields that appear further into the document.

The final online document then needs to be printable – but not in its entirety – just a summary that does not include all of the explanatory “how to” notes – Just the main headings and all of the answers.

Has anyone got any ideas or seen any examples like this?

I’ve used SurveyMonkey years ago and you can download excel spreadsheets that can be used in analysis.

Thanks for your responses everyone – I know it was a vague question -  After a little more investigation I think it’s clear that it will be a custom build – I have to finish the questions/document which is getting bigger by the day – I will then refine it and par it back.
It’s basically an online Marketing Questionnaire which gives the customer the guidance and steps to create their own Marketing Strategy & Plan online.

I’ll let you know when I’ve finished it if you’re curious.
