@robinoshaughnessy seems like there’s no other option, but have you tried to write down questions using facebook’s native Poll app? I mean have you written question there and have recognized whether this app allows you to put full question? This app is a poll app where you can write your question, then write answers or options and visitors/fans need to choose option from the given list. You can also allow fans to add option or stop them from adding option. This app will show you who chose which option. It will show fb username (with fb profile link) just next to option chosen.

@robinoshaughnessy oh, i got it now. Actually the requirements are really very rare to meet with any app. But still we can search for any such app. Or another way (if you and your client can afford), such app can be developed by any professional facebook app development company or a freelance developer. 

I don’t know of any quiz apps that would do exactly what you need, @robinoshaughnessy – most of them are social fun quizes and not as an entry mechanism.  You may check out  http://www.strutta.com/#showcase - their form app may fill the need.  Strutta is also really good with customizing something – it will be more expensive but at this point you are probably not going to find something out of the box.

i made it work in the end. created a image with the contest questions and then used North Social’s contact form for the people to enter their answers. 

I do wish that North Contact allowed for more than 20 characters in their text fields. 

This situation also made for a great conversation with the client about future competitions.
