I have the updated 3.4.1 version using the 2011 theme in WP. My HTML tab will not activate. I also cannot add any media.
Do I need to re-update? Any suggestions?

@deereinhardt For whatever reason, this seems to happen on some clients’ blogs and not on others. My problem is that I lose the formatting bar as well as the media upload button.
If you’re running any javascript or ad blockers, make sure you whitelist your blog’s domain. That may do the trick. It may also be a theme issue. Try switching themes temporarily to see if that does anything.

@deereinhardt I can find lots of articles about the Visual Editor disappearing – one is here - http://itdrag.com/2012/07/05/wordpress-update-3-4-1-missing-visual-editor-white-text-fixed/. Plus there is this one - http://wordpress.org/support/topic/cannot-disable-visual-editor-after-upgrade-to-341 - though I can’t tell if he wants to get rid of visual or HTML.
You might want to check your user profile in the WordPress dashboard – see if it has a preference checked for enabling or disabling the Visual Editor. Maybe if you disable the Visual, the HTML will show again (though I can’t be sure).

I host my site on GoDaddy. I called them. They suggested that I deactivate all of my plugins, clear my cache and then activate my plug-ins. That allowed the HTML to return, the links and insert images to work again, but the CSS still seems to be off a bit.
I expect that there will be a 3.4.2 pretty quickly if this many people are having issues.

Also can try removing WP and putting in a clean install. That is my preferred, instead of an upgrade.

You can export all your WP pages and posts and then re-import it after the re-install. Typically, plugins are usually the culprits for such issues. If you need some help, let me know. I’d be happy to fix it for you.
