@Blair. I agree about Gravity Forms. Once you start, you’ll find excuses to build more forms it’s so easy.

Just to say Blair, I’ve just started to use this plugin too.  It has a different pricing structure and may suit you better.  

I wanted to find something I could recommend to people where there was a one off price (although as a developer I don’t like one off pricing – it doesn’t set a product up to be very sustainable), BUT, that said here is another very good forms plugin :   http://formidablepro.com/
